Community Health Worker Program
The Community Health Worker Program (CHWP) is designed to encourage pregnant women and families with young children to use primary health care. The goals are for women to have healthy pregnancies and healthy babies and for children to be up to date on immunizations, lead testing and well child visits. Our focus is to assist underserved and hard to reach families get the best level of healthcare, self-sufficiency and family functioning possible.
The CHWP assists families to:
- Obtain primary medical care
- Set and keep appointments
- Apply for Medicaid, Food Stamps or temporary assistance
- Improve parenting skills
- Continue or complete education
- Budget
- Obtain childcare
- Obtain housing
Services are provided through free, family focused home visits, education, outreach and case management.

Healthy Families
Healthy Families is a free and voluntary program to help families meet the challenges of parenting. The program's home based services start during pregnancy or at birth and, depending on the needs of the family, may continue for up to 5 years. New or expecting parent(s) may ask their doctor, nurse, WIC counselor, social worker for a referral to Healthy Families or call us directly and talk to staff members about the program.
First, the Family Resource Educator reviews your life history, needs, goals and thoughts about parenting to assist by linking families to services in the community as well as offer immediate referrals. Healthy Families may also offer the services of a Family Support Worker to those families who would benefit from ongoing home visits. Family Support Workers make home visits to promote positive parent child relationships, healthy childhood growth and development, as well as provide ongoing up to date information about pregnancy and parenting to help meet and overcome challenges. We also have the services of our Fatherhood Advocate and Certified Lactation Consultant. There are educational and social opportunities such as our monthly "Sign with your baby" class, the Annual Holiday party with presents from Santa, and Family Picnic and Graduation.
What does a Family Support Worker do?
- Supports and reinforces the positive interactions and bonding between parents and child.
- Provide parenting education to enhance the parent-child relationship and the home environment.
- Initiate and maintains regular, long-term relationships with the families.
- Provide family centered and strength based support to families.
- Helps families identify needs and strengths to plan individual short and long term goals.
- Assist in building the family support system.
- Make referrals to appropriate agencies.
- Assist in building the problem solving ability of families.
- Information on infant/child growth and development stages
- Techniques to promote development and assist with screening to identify possible delays.
- Promote methods of stress reduction and appropriate discipline measures.
- Answer questions about health and childcare, such as how to calm a fussy baby and know when the baby should see a doctor.
- Help parents understand their new and changing roles including the importance of fathers to their child.

Maternal Child Health Care
Maternal Child Health (MCH) Nurses provide home-based services to pregnant, postpartum and parenting teens, women and families who live in Oneida County . The visits are free, although we will bill insurance whenever possible.
What services does the MCH program provide?
During Pregnancy
Check BP and Weight
Provide information about:
> Healthy eating
> How the baby grows and develops
> Labor and delivery
> Breastfeeding
> How to quit smoking
After delivery:
Check the baby’s weight and development
Provide information about baby care and parenting
We also offer:
> Referrals to other agencies as needed (for example: Headstart, Early Intervention, WIC, etc.)
> Case management and patient advocacy
> Teaching and support for families of premature infants who may or may not have oxygen or apnea monitors
> Monitoring of babies who aren’t gaining weight well

Medicaid Obstetrical and Maternal Services (MOMS)
This program provides financial assistance for qualified pregnant women. These women can receive supportive services including nutrition and psychosocial assessment and counseling, health education and care coordination.

Injury Prevention Program
The injury prevention program goal is to educate the community through prevention. This program distributes free car seats and bicycle helmets to families in need. Education on the proper installation of car seats is given at the time the car seat is given. Bike rodeos are done throughout the county to educate children and their parents on the importance of bicycle helmets and periodic bicycle checks and repairs.

Child Health Promotion Specialist Program
Child Health Promotion Specialists are registered nurses who are available to offer over-the-phone health related resource and referral information to child care providers regarding their child care programs including:
annual review of immunization records
dental health and nutrition programs
infection control programs
lead tracking programs
health and safety related trainings

Cancer Services Program
The program provides no cost breast, cervical, and colorectal screening for uninsured or underinsured men and women in Oneida, Herkimer and Madison counties, who meet program criteria. Follow up care and case management is provided for those who need further services. Treatment money is also available to those eligible. A strong focus is placed on outreach and education to the community, and reaching those who need the services. The Partnership includes healthcare providers, local health departments, physicians, voluntary agencies and individuals from the community. Presentations are available upon request.
SUSAN G. KOMEN FOUNDATION GRANT
The purpose of this grant is to provide mammograms, ultrasounds, and surgical consultations for women under 40 who have no health insurance and provide services for women found ineligible for the Healthy Living Partnership program. Also provides for some additional support services such as transportation, translation, child care, etc.
